This product consists of one term (four months) of UHIP coverage for one dependant of a student that is not currently enrolled in academic studies at York University for medical reasons. Only students who have been pre-approved by Cowan to enroll in a second consecutive leave of absence term may purchase this product. 

The purchase of this product does not guarantee that Cowan will approve UHIP enrollment during a second leave of absence term. If Cowan agrees to enroll the UHIP member during a leave of absence, a contract must be signed prior to the issuance of the UHIP card.

Tax is included in the cost of the product.
